The Web Part cannot find an InfoPath form in the specified location. Either the location does not have an InfoPath form associated with it or it is on a different site collection. Modify the Web Part Properties and select a list or library on the current site.

Instructions
A slide of an arrow is taped onto the diffuser plate. By placing the diffuser plate in between the model eye and the light source, an inverted image of the arrow will appear on the model's retina. The image comes into focus when the diffusion plate is approx. 12-15 inches away from the eye. The light source needs to be approx. 10-12 inches from the plate. The camera is focused on the retina plate from the back and projected on the video projector or on the portable monitor. The whole apparatus sits on a 4 ft cart.
